Walk into most physical therapy clinics in New York City and you'll see the same scene: one therapist bouncing between three patients, aides running the exercises, and a 45-minute "session" that delivers maybe 12 minutes of actual clinical attention. It's the insurance model working exactly as designed — high volume, low margin, and a race to the next appointment.
A growing number of New Yorkers are opting out. They're choosing concierge physical therapy instead, and the reasons come down to time, outcomes, and what your recovery is actually worth.
Concierge physical therapy is a private-pay model built around one principle: one patient, one therapist, one full hour. At James Fowler Physical Therapy in Union Square, that means:
Insurance reimbursement rates for physical therapy have been declining for over a decade. To stay profitable, most clinics respond in the same ways: double- and triple-book patients, delegate hands-on work to unlicensed assistants, and shorten the licensed portion of the visit.
The result is care that's technically covered but rarely effective. Patients plateau, treatment stretches into 20 or 30 visits, and the underlying problem often comes back.
Concierge sessions cost more per visit — but almost every patient we see finishes care in fewer total visits. When a licensed clinician spends 60 uninterrupted minutes with you, progress compounds. Programs get updated in real time. Home exercises are actually taught, not handed out on a printed sheet.
For most patients, the total out-of-pocket cost is comparable or lower than an insurance-based plan once co-pays, deductibles, and lost work time are counted.
We're out-of-network by design, but we make reimbursement easy. Most commercial PPO plans reimburse a meaningful percentage of concierge visits when you submit a superbill. We provide the paperwork, the CPT codes, and the guidance so you can file with confidence.
Concierge PT isn't for everyone. If a basic exercise progression will get you where you need to go, insurance-based care can work. But if you have a stubborn injury, a demanding job, a competitive goal, or you've tried the standard model and stalled — the concierge experience is a different tier of care.
