What a Concierge Physical Therapy Session Actually Looks Like

"What do you actually do in an hour?" It's one of the most common questions we get from new concierge patients. The short answer: a lot more than a typical PT visit. The longer answer is below — a real look at how one of our 60-minute sessions is structured, and why each piece matters.

    Minutes 0–5: Check-In and Recap

You arrive at our Union Square clinic and go straight to your private treatment room — no waiting-room queue. Your Doctor of Physical Therapy is already reviewing your notes from the last session. We start by talking about what happened between visits: what felt better, what flared up, how the home program went, and any new pain or activity to know about.

This isn't small talk. It's data. Every answer shapes what we do for the next 55 minutes.

    Minutes 5–15: Movement Reassessment

Next we test the specific movements and structures we're tracking — range of motion, strength, balance, gait, sport-specific patterns. We measure. We compare to last session. We look for the small changes that tell us the program is working (or needs to change).

This is where continuity pays off. Because you see the same therapist every visit, we spot subtle shifts a rotating staff would miss.

Minutes 15–35: Manual Therapy and Hands-On Work

The middle of the hour is dedicated to skilled manual therapy — joint mobilization, soft-tissue work, myofascial release, and other hands-on interventions delivered by a licensed clinician. This is the part most insurance-based clinics compress or hand off to an aide. In a concierge session, it's the core.

Minutes 35–50: Guided Movement and Loading

Manual work opens the door. Movement walks you through it. We use the second half of the hour to load the tissue we just addressed — corrective exercises, strength progression, motor-control drills, and sport- or task-specific movement patterns. Every rep is coached in real time.

If you're a runner, we might film your gait. If you're recovering from a shoulder surgery, we're progressing the exact ranges your surgeon cleared. If you're a golfer, we're building the rotational patterns your swing depends on.

Minutes 50–60: Home Program and Next Steps

The last ten minutes are for teaching — not printing. We film your home exercises on your phone so you have your therapist demonstrating each movement. We answer questions. We tell you exactly what to do, what to watch for, and when to reach out if something changes. You leave with a plan you can actually execute.

    Between Sessions

Concierge care doesn't stop at the door. You have direct-message access to your PT for questions during the week — a flare-up, a form check, a question about a workout. That kind of access catches problems early and keeps you moving forward.

    Why the Structure Matters

A full hour of one-on-one care isn't a luxury — it's what evidence-based physical therapy actually requires. Manual therapy needs time. Motor learning needs repetition. Progression needs measurement. When any of those get squeezed out to keep a clinic on schedule, outcomes suffer.
